Do I need an appointment?

Yes. Currently, we are not taking any walk-ins. The best way to reach us to get an appointment is by calling us at 904.900.3472. You may also ask us to call you back via text message on the same number or via e-mail at contactus@cornerstonefamilymedicinellc.com.
Am I covered by insurance?

List of insurances we take: Tricare, Ambetter, Medicare, Florida Blue, Aetna, Cigna, United Healthcare and Mayo.
Do I need a medical history?

If you are a new patient, you can bring your medical records on your first visit. We can also request your medical history from your previous Primary Care Provider after you sign the Medical Records Request form.
What information should I bring?

Always bring your proof of identification (ID) and insurance card whenever you visit.
